A stuntwoman was killed in a motorcycle crash on the set of Deadpool 2 in Vancouver on Monday. The victim’s identity has not yet been released.

Filming for the second Deadpool movie, starring Ryan Reynolds, began in Canada in June and has continued throughout the summer. On Monday morning, shooting was taking place by Shaw Tower and Jack Poole Plaza when the fatal accident occurred. The stuntwoman was filming a motorcycle sequence when she apparently lost control of the vehicle and crashed into one of the tower’s glass windows. Tragically, the woman was pronounced dead at the scene.

Vancouver Police confirmed on Twitter, “A stunt driver has died on the set of Deadpool during a stunt on a motorcycle. VPD & @WorkSafeBC investigators are at the scene.” Work Safe BC is a Canadian organization that monitors workplace injuries as part of the Workers’ Compensation Board Of British Columbia. As of yet, there has been no public statement from Marvel or 20th Century Fox, and it’s unclear if Reynolds was on set at the time. Production was expected to continue in the Canadian city through early October, and it’s unclear how this tragic incident will affect the schedule.

This is the second instance of an on-set stuntperson death in the last four weeks. Almost exactly a month ago, stuntman John Bernecker died after a fall on the set of “The Walking Dead.” He was attempting to do a choreographed fall from a makeshift balcony when something went wrong, causing him to lose his balance and land on the concrete floor below, instead of the crash pad that was laid out for him.

And just this past weekend, Tom Cruise was seemingly injured while attempting to film a stunt for Mission Impossible 6. These accidents come admit calls for greater safety for stunt professionals in the film and TV industries. UPDATE: 20th Century Fox says in a statement, “We are deeply saddened by the accident that occurred on the set of Deadpool 2 this morning. Our hearts and prayers are with the family, friends and colleagues of our crew member during this difficult time.”

Reynolds also mourned the stuntwoman’s death on Twitter later on Monday. “Today, we tragically lost a member of our crew while filming ‘Deadpool,’” the actor wrote in a statement. “We’re heartbroken, shocked and devastated… but recognize nothing can come close to the grief and inexplicable pain her family and loved ones must feel in this moment. My heart pours out to them — along with each and every person she touched in this world.”
